Raised in Arizona, Dr. Carlson moved to Colorado to begin his Family Medicine training in 2003. He completed a fellowship in Sports Medicine in 2007 where he served as a team physician for the
University of Colorado Buffaloes and the University of Denver Pioneers. He practiced rural medicine before beginning full-time outpatient primary care in Denver, where he has practiced since 2008.
Dr. Carlson enjoys the primary care setting as it allows for the establishment and development of individual and family relationships over time. When not working with patients, Dr. Carlson spends his
time reading, cycling, cooking, skiing, and hiking with his wife and dog. He is a music collector and B-movie hobbyist.

Scott Dorn
“My draw toward working with seniors is personal, and when I’m around older patients, I just feel at ease. I’m passionate about giving seniors the care they need, and being around them puts me in a comfort zone that’s full of joy and clarity.”
Raised by his grandparents, Scott Dorn became interested in senior care early in his life. Attending appointments with his family sparked his curiosity in their health and inspired him to pursue senior health care as an adult. “I always want to treat patients as well as I’d treat my own family. In this way, they become a part of my extended family,” he says.
Scott received his BA from the University of Rhode Island and also holds a Masters of Science in traditional Chinese medicine as well as a Masters of Medical Science. Scott has lived in the Denver area since 2006, and he worked a rotation in geriatrics with IPC before coming to work as a certified physician assistant for IPC/SCOC in 2015.
“I view my experience in Chinese medicine and acupuncture as complementary to my current work,” he says. “It helped me understand conditions that commonly affect seniors, including stress, fatigue, inflammation, and arthritis.”
When he’s not meeting with patients, Scott enjoys hiking, skiing, and spending time with his wife, two children, and grandfather, who continues to inspire his medical practice.

Erik Miller
Erik Miller PA-C is from a small community in the four corners area. Coming from a family of healthcare workers, his curiosity of medicine started at a young age. His passion grew after witnessing the positive change his own family provider gave to others. Specifically, Erik was drawn to family medicine to forge continued bonds with his patients and give the best individual care to each. The forefront of Erik’s philosophy is treating the overall health of a person and not just their illness. Erik received his Bachelor of Science from the University of New Mexico and shortly after attended the University of St. Francis where he completed his master’s in physician assistant studies. Prior to becoming a certified PA, Erik gained healthcare experience in primary care, urgent care, and emergency medicine. When he’s not meeting with patients, Erik enjoys skiing, watching sports, traveling, and exploring new food options in the Denver.
Neila Ray
Neila Ray, FNP-C has over seven years of nursing experience. She has a Bachelor of science in nursing from Lincoln Memorial University and a Masters of science in nursing from Lincoln Memorial university.
“I genuinely enjoy taking care of people. After spending time in a corporate setting, I felt unfulfilled and knew I needed a change. Nursing was a field that is passion and care driven and changing my career path was the best decision I ever made.

Neila loves the outdoors. She is an avid fan of hiking, kayaking and biking. She is originally from Ohio, but recently relocated from Tennessee to Colorado.

Sarah Wierda
Sarah Wierda FNP-C is passionate about providing illness prevention, safety, and healthy lifestyle education for her patients with compassion and with a holistic approach. She received her Bachelor of Science in Nursing in 2010 at the University of Colorado, Colorado Springs and worked many years in ENT and Allergy. She later returned to the University of Colorado, Colorado Springs to complete her Family Nurse Practitioner program. Sarah is a native of Colorado and is a busy Mom of twins!
She enjoys reading and spending time with her husband and kiddos in the mountains, Gunnison being a particular favorite. Like many Colorado natives, she has a passion for watching football (go Broncos!) and basketball (go Nuggets!) games as well.

Allison Corzatt
Allison Corzatt is a board-certified Family Nurse Practitioner (FNP-BC). She is passionate about Preventative Medicine and Women’s Health. As part of the team at Columbine Family Practice she wants to focus on building relationships with individuals and their families, while partnering with each patient in their healthcare journey. She encourages her patients to make healthy lifestyle changes every day, always focusing on improving their well-being. Making those changes isn’t always easy, but she wants to be an advocate and support for her patients to be able to achieve their healthcare goals.
Allison has been in the nursing field for over 12 years, most spent in Critical Care. She received her Bachelor of Science from Lynchburg University in Virginia and her Master of Science from Simmons University in Massachusetts.
In her spare time, she loves traveling, camping, and hiking with her husband and dog. She also enjoys reading, cooking, and writing.
